Name change checklist
There is no single form for changing your name after a wedding in Ireland. You tell each organisation yourself. Tick what applies to you and work down your own list, in the order that saves going back twice.
The names are optional and only fill in the letter below. Nothing is sent anywhere: it all stays in this browser.

First, your proof
Make sure the marriage is registered
Official ruleAfter a civil ceremony the registrar does it. After a religious or secular ceremony you return the signed Marriage Registration Form to a Registrar within 1 month. No certificate can be issued until then.
Order your marriage certificate
Official ruleA full standard certificate costs €20. Order online, by post, by phone or in person at any Civil Registration Service. The Passport Service asks for the original, so think about a second copy for everything else.
Money and work
Banks, credit union and cards
Every account, card and loan. Banks usually want to see ID in the new name, so do them after your passport or licence.
Home and car
Electricity, gas, broadband, phone and TV licence
Most can be changed in your online account or with one call.
Health
GP, dentist and pharmacy
And any hospital or clinic you attend, so records and prescriptions match.
Everything else
Email, loyalty cards and subscriptions
Last and least urgent. Do them as they come up.
A letter you can send to most of them
For banks, insurers, utilities and anyone else without a form of their own. Fill in your names above and it is written for you. Paste it into an email or print it.

The questions everyone asks
What do I need to change my name after marriage in Ireland?
Your marriage certificate. Most organisations accept it as proof, and you do not need a deed poll. A full standard certificate costs €20.
What should I change first?
Get the marriage certificate, then change photo identification such as your passport or Public Services Card. Banks and other organisations usually ask for ID in the new name, so they come after.
How do I change the name on my Irish passport after marriage?
Apply for a new passport in your married name and submit your original civil marriage certificate. If you have been issued a passport in your birth name since the wedding, you also need two years' proof of using the new name.
How do I change the name on my Public Services Card?
Attend a SAFE Registration Centre in person, as a new signature is needed. Bring photo ID in your pre-marriage name and your marriage certificate.
Not sure you want to change it at all? You don't have to, and there is no deadline.
Where the rules come from
Steps marked "Official rule" state what these pages said on 13 October 2026. The other steps are reminders of who to tell; each of those organisations sets its own requirements, so check with it first.
